A decision log can become a list of conclusions that no longer explain the problem. This workshop asks a group to record a decision together with the alternatives it considered, the constraint that mattered and the question that remains open. The point is not to make a comprehensive archive of every conversation.

The exercise then passes the record to another participant, who describes what they can do with it and what is still missing. That exchange makes the handover tangible without treating a template as a substitute for judgement.
